Patient Registration

We are happy to register patients who live within our practice boundary. For information on whether you live within the boundary, please contact the reception staff who will be happy to assist with your query or click on the link below.
To register, please download, print and complete the form below, for each family member. Alternatively, please come into the practice and we will provide you with a paper copy of the forms. We require you to also complete our New Patient Questionnaire which can also be accessed using the undernoted link.
You will be invited to attend for a new patient medical appointment on registration.
Please notify the practice of any change in contact details so that we can keep your records up-to-date.
If you relocate outwith the practice boundary it may be necessary for you to register with a new practice.

Patient Confidentiality
Your Medical Records are confidential. Under no circumstances is information about you shared with third parties who do not directly contribute to or support the delivery and planning of your health care, unless your consent has been obtained.
Routine appointments
Consultations are by appointment only and are available from 09:00-12:00 and from 15:30-17:00. Extended hours appointments are available for working patients between 08:00 – 09:00 on selected mornings – these are limited and can be requested through our reception staff.
Routine nurse appointments are available every day between 09:00 and 17:00.
Phlebotomy clinics run Mon – Thurs 09:00 – 13:00.
Urgent calls and requests are dealt with through our triage system, led by our Advanced Clinical Practitioner Sharon Hamid. If you request an urgent appointment you will be called back by our triage team and an appointment may be allocated, your issue dealt with over the telephone or you may be directed to an appropriate allied health professional.
Do not walk in to the Practice and expect to be seen instantly. The Practice works on an appointment only system – be prepared to be asked to return or please phone the reception team who will be more than happy to allocate you an appointment time. Urgent and emergency situations will be dealt with accordingly.
Please arrive early to your appointment, it may not be possible for you to be seen if you are late. Patients attending more than 10 minutes late will be automatically asked to reschedule. If you wish to reschedule or cancel an appointment, please let us know so that it can be offered to another patient.
Out of Hours
Patients with an urgent problem out of hours are asked to telephone 111, free of charge. Please click on the link for further information. In the event of a life threatening emergency dial 999.
Missed Appointments
Missed appointments waste practice time and prevent us from seeing patients. If you do not attend 3 appointments within a 3 month period you will receive a letter from the practice. You will not be able to book any further routine appointments without prior discussion with the Practice Manager. Continual non-attendance may risk removal from our practice list.
Suggestions and Complaints
We strive to provide a quality service to our patients. We welcome any suggestions or comments to improve this service.

Online Appointment and Repeat Facility
We offer online Patient Access, giving patients the opportunity to book a doctors appointment online as well as requesting repeat prescriptions. If you are interested in registering for this new service please click on the button below to download and complete the information required and return to the Practice. A registration information letter will be made available to you to enable you to set up your online booking account.
Zero Tolerance
This Practice supports the Scottish Executives policy on zero tolerance.
For the sake of our staff and other patients we will not tolerate any verbal aggression either within the practice, on the telephone or in the community.
Should a patient be violent, abusive or aggressive to a doctor or any member of their staff, we will ask that their name be removed from our list of patients with immediate effect.
